Step 1:
Choose the office location below.
Step 2:
Choose the type of appointment. If you select an appointment type that will not give you appointment time options, it means that appointment type is not available for online appointment scheduling at your office of choice and/ or with that provider.
Step 3:
Choose the audiologist.
Step 4:
The calendar will appear with available time slots for the audiologist you choose. Click on your date of choice.
Step 5:
A list of available time slots will appear at right. Choose your time slot.
Step 6:
Complete the brief form and choose the Schedule Appointment button at the bottom.
Step 7:
A staff member from your office of choice will contact you to gather additional information needed before your appointment.
Please note: Cochlear Implant Evaluations are available in the Chattanooga office location only.
